New Patients
We take new patients of every age, newborns included. Come 15 minutes early to your first visit, or fill out the forms at home from the link we text you.
Bring These to Your First Visit
- Your insurance card and a photo ID
- Every medicine you take, or just bring the bottles
- Your child's shot record
- The name of your last doctor, so we can ask for your records
- School or sports forms that need a signature
- A list of questions, if you have them
Your First Visit
Forms
We text you a link after you request the visit. It takes about 10 minutes.
Check In
Show your card and ID at the front desk. We copy them once.
Vitals
A medical assistant checks weight, blood pressure and your medicines.
Your Doctor
First visits are booked for 40 minutes, so there is time for your whole history.
Moving Your Records
Sign one release at check in and we ask your last office for your records. Most arrive within two weeks. You don't have to carry anything.
